As the legal parent or guardian, I release and hold harmless Learning Together AV, Inc., its owners and operators from  and all liability, claims, demands and causes of action whatsoever, arising our of or related to any loss, damage, or injury including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Learning Together AV, Inc., its owners and operators or in route to or from any of said premises, except due to negligence or willful dangerous acts by staff.

The undersigned gives permission to Learning Together AV, Inc., its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian.  I hereby declare any physical/mental problems, restrictions or condition and/or declare the participant to be in good physical and mental health. I request that our doctor/physician listed on file be called and that my child be transported to requested hospital. (Please provide physicians' phone number.)

I have no objection to my child being included in photographs, slides or movies taken at Learning Together AV, Inc., that might be used for purposes of interpreting the program or for program publicity. (If you have an objection, please send an email to learningtogetherav@gmail.com and we will send you an exclusion form).

I understand that if less than 3 students sign up for an individual enrichment class, that enrichment class may be cancelled at the discretion of Learning Together AV, Inc.

I understand that Learning Together AV, Inc. offers enrichment classes and tutoring.  Homeschool parents and caregivers are fully responsible for their student's academic progress. 

​

I understand that after consultation with the teacher of record my child may be discharged from any program would I, or my child, fail to comply with the requirements or the policies, or the appropriate requirements, of the management personnel of Learning Together AV, Inc.

I understand that enrichment classes are currently in-person. Learning Together AV, Inc. will continue to follow guidelines put in place by Charter schools and our local governments to remain in-person.  In the event that we must return to a virtual classroom situation tuition will not be refunded and supplies become the responsibility of the parent. 

 

I understand that if any member of my household tests positive for Covid-19 I will let staff know as soon as possible.

I understand that if an enrichment class is not dropped by me in writing by the end of the second week of classes, I am committed to pay for the entire session.
